Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 year of experience required License: Valid state nursing license required in-hand when applying Certifications: All required certifications must be in-hand when applying Must-Have: - CPI Blue Card from the Crisis Prevention Institute required in-hand when applying - Experience with charting systems required - EPIC experience must be noted in work history - Two supervisor references required when applying Description: The Case Manager primarily focuses on inpatient cases. There may be occasional cross-cover to the emergency department for discharge planning and admit utilization management. This position is Monday through Friday, with a modified schedule from 8:30 AM to 5:00 PM. Previous charge experience is preferred. Experience in care coordination and discharge planning is essential. Candidates should be knowledgeable regarding admission criteria and CMS guidelines. Familiarity with DRG, HIPAA guidelines, and needs assessment/order durable medical equipment is important. The candidate will work within interdisciplinary support services, such as interpretation services and social services. Patient age groups include adolescents, adults, and geriatrics. Compliance with no background hits is mandatory. A maximum of 7 days of requested time off per contract is highly preferred. All requested time off must be included in the application and cannot be modified once submitted. Local candidates are accepted at the same rate. Candidates must have experience working in an acute care hospital setting. Returning staff must be gone for a minimum of one year to be eligible to return as a traveler. Manager interviews may or may not take place. Modules are part of the training process and vary by specialty.
