Job Title: Level 2 Registered Nurse Case Manager

Location: New York, NY

Pay: $2,685.60/week

Shifts: Monday - Friday, Day shift 8:00am-4:30pm, weekends per need

Seven Healthcare are seeking a dedicated Level 2 Registered Nurse Case Manager with expertise in case management for a rewarding travel assignment in New York, NY. The role focuses on delivering exceptional patient care in a home health setting, utilizing skills in EPIC or Netsmart, OASIS, and Telemetry.

Key Responsibilities of the Level 2 Registered Nurse Case Manager
  • Develops and revises plans of treatment and discharge plans with patients, families, and healthcare teams
  • Completes and submits OASIS assessments and related documentation
  • Processes orders and communicates patient needs and changes to the physician
  • Consults with and educates patients and families on disease processes and self-care techniques
  • Coordinates and supervises the quality of patient care services provided by the agency
  • Participates in agency operational and financial objectives by monitoring quality indicators and productivity
  • Contributes to an environment of professional growth and competency

Requirements of the Level 2 Registered Nurse Case Manager
  • Certified Home Health Care Experience required
  • Experience with EPIC or Netsmart
  • OASIS and Telemetry experience required
  • Current RN License (NY License ONLY)
  • BLS Certification
  • One year of experience as a clinical nurse in a hospital
  • Critical thinking and communication skills
  • Ability to work independently and manage various assignments
  • Current NYS driver's license
