Job Title: Care Manager Profession: RN Specialty: Care Management Duration: 13 weeks Shift: 8 hours Hours per Shift: 8 Experience: Minimum 3 years; Preferred 5+ years with UM and CM License: BSN Certifications: Required certification in nursing; preferred certification in CCM or a related field Description: The Care Manager will utilize a collaborative process to assess, plan, implement, monitor, and evaluate options/services required to meet a patient’s health needs. Effective communication and available resources will be used to promote quality and cost-effective outcomes for 18-22 patients. The Care Manager will work to meet patient needs and manage length of stay. Efficient utilization of resources is essential. The role includes determining medical necessity and appropriateness of admission using screening criteria. Knowledge of benefits, plan contracts, eligibility issues, and requirements is necessary. Extensive background and knowledge of current practice patterns and acute inpatient care are required. Additionally, methods for assessing physical and mental impairment, as well as understanding the characteristics of illness and expected treatment, are necessary competencies.
