They collaborate and communicate with interdisciplinary teams in all phases of discharge and care planning processes. Effective communication with patients, families, medical staff, care team members, students, and supervisors is essential. Evaluation of the process of initiating plans for discharge and/or transitions of care and ongoing case management needs for patients and/or families is required. They supervise staff in the coordination and facilitation of patient care progression throughout the continuum of care. The role includes participating in the interviewing and hiring process by reviewing job profiles, applications, and resumes. They develop behavioral interview questions, conduct interviews, and provide feedback and recommendations on hiring decisions. Ensuring that referrals are initiated to the appropriate next level of care in a timely manner is critical. Coordination of ordering of medical equipment and supplies for safe discharge planning or ongoing care planning is required. The Manager works with staff to facilitate family meetings for all patients as requested by patients, families, or healthcare teams. They lead collaboration with physicians and members of interdisciplinary teams to facilitate care for designated case loads. The role supports contact with families, physicians, or other healthcare team members in anticipation of patients' or families' special needs, ensuring timely facilitation of effective treatment plans. Providing educational materials and methods in support of team members’ ability to document appropriately to patients' levels of understanding and motivation is necessary. They ensure that patients and families are appropriately assessed for needs, developing individualized continuing care plans in collaboration with physicians. The Manager educates staff on variances in patient responses and how to proactively adjust care plans in coordination with the interdisciplinary team. Supervision of staff on the screening process with patients within 24 hours of admission for appropriate services and interventions is required. Evaluating team members under supervision ensures proper knowledge gaps are closed and necessary education is provided. The Manager participates in the evaluation of metrics and collaborates with case managers under supervision to achieve operational goals and metrics. They understand how to analyze individual productivity and evaluate length of stay on assigned units with case managers. Proactive collaboration with clinical directors for discharge planning is essential. Ensuring equitable staffing for all units and daily facility coverage is necessary. They may be expected to be on call during weekends and holidays. There is a possibility of floating between areas. Epic access is needed for case management modules.
