Enjoy broad independence while being part of a cohesive interdisciplinary team. Achieve a work/life balance that matches the flexibility needed. Focus on providing pain management and comfort for patients with terminal or chronic illness. Communicate and coordinate care needs with the interdisciplinary team, patients, and their families/caregivers. Develop a care plan in collaboration with the patient's physician, the patient, and their family or primary caregivers upon admission. Periodically review the patient care plan and recertify it as appropriate. Coordinate the patient's care plan through interdisciplinary communication and case conferences when necessary. Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems or progress toward desired goals. Communicate significant changes to the patient's physician and colleagues. Document evaluations, patient responses, and progress toward goals. Teach the patient and caregiver care techniques and end-of-life symptom management as appropriate.
