Job Title: RN - Case Management - Travel

Location: Waterford, WI

Pay: $2,546.40 weekly

Shifts: M-F 8am-5pm (No on-call)

Seven Healthcare are seeking an RN for a travel contract in Waterford, WI, focused on case management with a specialization in hospice care. The successful candidate will work in a dynamic and fast-paced environment, handling 5 visits per day without case management or IDG responsibilities.

Key Responsibilities of the RN - Case Management - Travel
  • Perform comprehensive assessment of high-risk members
  • Collaborate with primary care providers for individualized care plans
  • Address and reassess cost-efficient, appropriate levels of care
  • Put non-medical support in place for treatment compliance
  • Coordinate care transitions as needed
  • Coordinate with physicians, social workers, and claims professionals
  • Engage specialty resources for optimal resolution
  • Maintain detailed records of clinical and fiscal outcomes
  • Identify opportunities for health promotion and illness prevention
  • Act as patient advocate, ensuring privacy and confidentiality

Requirements of the RN - Case Management - Travel
  • Associates or Bachelor's Degree in Nursing or related field
  • Current, unrestricted state Registered Nurse license
  • Hospice and wound care experience
  • Experience with HCHB, Matrix Care, or any Electronic Medical Record system
  • Certified CPR/BLS
  • Valid Driver's License and reliable vehicle with insurance
  • Skills competency checklist and training documents for compliance
  • Case Management certification preferred
