Job Title: RN - Case Management - Travel

Location: Waterford, WI

Pay: $2,546.40 weekly

Shifts: M-F 8am-5pm (No oncall)

Key Responsibilities of the RN - Case Management - Travel
  • Perform comprehensive assessment of high-risk members
  • Collaborate with primary care providers for individualized care plans
  • Ensure cost-efficient, appropriate levels of care
  • Coordinate non-medical support for treatment plan compliance
  • Manage care transitions and engage with necessary specialties
  • Keep detailed records of clinical, functional, and fiscal outcomes
  • Promote health and prevent illness where possible
  • Act as patient advocate, protecting privacy and confidentiality

Requirements of the RN - Case Management - Travel
  • RN License for WI
  • Hospice experience, Wound Care Experience
  • Experience with Home Care Home Base-HCHB or similar EMR
  • Certified CPR/BLS
  • Valid DL, Reliable Vehicle, Valid Car Insurance
  • Case Management certification
  • Associates or Bachelor's Degree in Nursing or related field
  • Skills competency checklist including Elder Abuse, HIPAA, Infection prevention, OSHA, and recent education/skills training
