Job Title: RN - Case Management - Travel

Location: Waterford, WI

Pay: $2,546.40 weekly

Shifts: M-F 8am-5pm (No oncall)

Seven Healthcare are seeking a dedicated RN - Case Management - Travel professional in Waterford, WI. This role requires expertise in hospice care, wound care, and visits - no case management or IDG. Candidates will perform comprehensive assessments and collaborate for individualized care plans in a supportive work environment.

Key Responsibilities of the RN - Case Management - Travel
  • Perform comprehensive assessment of high-risk members.
  • Collaborate with primary care providers for individualized care plans.
  • Address and reassess cost-efficient, appropriate levels of care.
  • Put non-medical support in place for treatment plan compliance.
  • Coordinate care transitions as needed.
  • Coordinate with physicians, social workers, and others for patient care.
  • Engage specialty resources as needed.
  • Keep detailed records of clinical, functional, and fiscal outcomes.
  • Identify opportunities for health promotion and illness prevention.
  • Act as a patient advocate protecting privacy and confidentiality.

Requirements of the RN - Case Management - Travel
  • RN License for WI
  • Hospice experience
  • Wound Care Experience
  • Home Care Home Base (HCHB) experience preferred; Matrix Care or any EMR acceptable
  • Certified CPR/BLS
  • Valid Driver's License
  • Reliable Vehicle with Valid Car Insurance
  • Skills competency checklist
  • Training in elder abuse, HIPAA, infection control, and OSHA
  • Associates or Bachelor's Degree in Nursing or related field
  • Current, unrestricted state Registered Nurse license
  • Case Management certification
