Job Title: RN - Case Management

Location: Savannah, GA

Pay: $2,546.40 per week

Shifts: Monday to Friday, 8:30 AM - 5:00 PM, with on-call duties 6 days per month (including 2 weekends)

Key Responsibilities of the RN - Case Management
  • Perform comprehensive assessments of high-risk members
  • Collaborate with primary care providers to develop individualized care plans

Requirements of the RN - Case Management
  • Associates or Bachelor's Degree in Nursing or related field
  • Current, unrestricted GA RN license
  • Case Management certification
  • Proficiency in OASIS documentation and accuracy
  • Knowledge and use of HCHB
  • Experience in home health care
