Job Title: RN - Case Management

Location: Pensacola, FL

Pay: $2,546.40 weekly

Shifts: M-F 8am-5pm (Some weekend availability would be great- 1 weekend per month/1 on call night per week)

Seven Healthcare are seeking a dedicated RN specializing in Case Management for an exciting travel contract in Pensacola, Florida. This role involves providing expert care to patients in a supportive and collaborative work environment, covering counties including Escambia, Santa Rosa, Okaloosa, and Walton.

Key Responsibilities of the RN - Case Management
  • Perform comprehensive assessment of high-risk members
  • Collaborate with primary care providers to create and implement individualized care plans
  • Evaluate and reassess cost-efficient and appropriate levels of care
  • Coordinate non-medical support to ensure compliance with treatment plans
  • Coordinate with healthcare professionals for appropriate patient transition
  • Engage specialty resources as necessary
  • Maintain detailed clinical, functional, and fiscal records during management
  • Identify opportunities for health promotion and illness prevention
  • Act as a patient advocate, addressing privacy and confidentiality issues

Requirements of the RN - Case Management
  • Associates or Bachelor Degree in Nursing or related field
  • Current, unrestricted state Registered Nurse license (Florida/Compact)
  • Proficiency in OASIS/HCHB is a must
  • Experience in wound care, PICC/IV care, lab collection, and post-surgical care is preferred
