Pay: $2,546.40 weekly

Shifts: Monday through Friday, 8:00 am to 4:30 pm, with on-call rotation

The role involves traveling across Coffee, Franklin, Grundy, and Van Buren counties, providing care and support for patients in their homes.

Key Responsibilities of the RN: Case Manager
  • Perform comprehensive assessment of high-risk members.
  • Collaborate with primary care providers to implement individualized care plans.
  • Ensure cost-efficient and appropriate levels of care.
  • Coordinate non-medical support for treatment plan compliance (housing, transportation).
  • Facilitate care transitions and coordinate with care teams.
  • Engage specialty resources to achieve optimal resolution.
  • Maintain detailed records of outcomes during management process.
  • Identify opportunities for health promotion and illness prevention.
  • Act as patient advocate by ensuring privacy and confidentiality.

Requirements of the RN: Case Manager
  • Associates or Bachelor's Degree in Nursing or related field.
  • Current, unrestricted state Registered Nurse license.
  • CPR/BLS License
  • Valid driver's license and insurance.
  • Ability to analyze complex medical information and make rational decisions.
  • Case Management certification preferred.
