Job Title: RN - Case Management - Travel

Location: Lexington, KY

Pay: $2,546.40 weekly

Shifts: Monday through Friday from 8am-5pm, On call one weekend a month, one day each week

Seven Healthcare are seeking an experienced RN in Case Management for a travel assignment in Lexington, KY. This role focuses on providing home health services, utilizing your expertise in OASIS and HCHB to ensure quality care. This position involves covering multiple counties, offering a dynamic and engaging work environment.

Key Responsibilities of the RN - Case Management - Travel
  • Perform comprehensive assessments of high-risk members.
  • Collaborate with primary care providers to implement individualized care plans.
  • Address and reassess cost-efficient, appropriate levels of care.
  • Coordinate non-medical support to ensure compliance with treatment plans.
  • Manage care transitions and coordinate with relevant professionals.
  • Engage specialty resources for optimal patient resolutions.
  • Maintain detailed records of clinical and fiscal outcomes.
  • Identify opportunities for health promotion and prevention.
  • Advocate for patient privacy and confidentiality.

Requirements of the RN - Case Management - Travel
  • Associates or Bachelor's Degree in Nursing or related field.
  • Current, unrestricted state Registered Nurse license.
  • Case Management certification.
  • Home Health Experience, OASIS, and HCHB skills.
  • Kentucky license or compact license.
