Job Title: RN - Case Management

Location: Jeffersonville, IN

Pay: $2,546.40 weekly

Shifts: M-F 8am-5pm, with required weekend on-call rotation

Seven Healthcare are seeking an experienced RN - Case Management professional with a focus on home health. This travel contract role is based in Jeffersonville, IN, and offers a dynamic work environment across Bullitt and South Jefferson counties, covering other areas as needed for PTO, call-ins, and admissions.

Key Responsibilities of the RN - Case Management
  • Perform comprehensive assessment of high-risk members.
  • Collaborate with primary care providers to implement care plans.
  • Identify and reassess cost-efficient levels of care.
  • Coordinate non-medical support to ensure treatment compliance.
  • Coordinate care transitions with relevant professionals.
  • Engage specialty resources for optimal resolution.
  • Maintain detailed records of clinical and fiscal outcomes.
  • Identify health promotion and illness prevention opportunities.
  • Act as a patient advocate by protecting privacy and confidentiality.

Requirements of the RN - Case Management
  • Associates or Bachelor's Degree in Nursing or related field.
  • Current, unrestricted state Registered Nurse license.
  • Ability to analyze complex medical information and make rational decisions.
  • Two years of recent experience in home health, wound care, IV, drain care, and G tubes.
  • HCHB experience preferred.
  • RN Licensure in KY or compact license accepted.
