Shifts: Monday to Friday, 8 AM - 5 PM; On-call every other weekend

This role involves working onsite and engaging directly with patients in various settings. Key Responsibilities of the RN Case Manager- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers for comprehensive care plans
- Identify and reassess appropriate levels of care
- Coordinate non-medical support for treatment compliance
- Facilitate care transitions with multidisciplinary teams
- Engage specialty resources to achieve optimal outcomes
- Document clinical, functional, and fiscal outcomes accurately
- Promote health and illness prevention strategies
- Act as a patient advocate, ensuring privacy and confidentiality
Requirements of the RN Case Manager- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted RN license in Alabama
- Case Management certification
- Ability to analyze complex medical information
- Experience with home healthcare, OASIS, and HCHB
