Job Title: RN - Case Management - Travel

Location: Brunswick, GA

Pay: $2,546.40 per week

Shifts: Monday-Friday with on call, 8 am to 5 pm (On call once every three weeks)

This is a travel assignment within a diverse community, covering Glynn, Camden, Brantley, and Charlton counties. The role requires a skilled nurse with expertise in wound care, IV care, and home health experience, who is committed to high-quality patient care.

Key Responsibilities of the RN - Case Management - Travel
  • Perform comprehensive assessment of high-risk members
  • Collaborate with primary care providers to ensure the implementation of individualized care plans
  • Continuously reassess and address cost-efficient, appropriate levels of care
  • Coordinate non-medical support to ensure compliance with treatment plans
  • Coordinate care transitions and engage specialty resources as needed
  • Maintain detailed records of clinical, functional, and fiscal outcomes
  • Act as patient advocate by protecting privacy and confidentiality issues

Requirements of the RN - Case Management - Travel
  • GA RN License, willing to accept compact license
  • Associates or Bachelor's Degree in Nursing or related field
  • Case Management certification
  • Experience in wound care, IV care, and home health
  • Ability to analyze complex medical information and make rational decisions
  • HCHB experience
