Care Manager RN Registered Nurse Case Manager Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 16:30 Experience: Minimum 3 years of inpatient care management experience License: Current RN license Certifications: BLS, Case Management Certification Must-Have: Recent acute care experience in hospital care management/discharge planning Ability to float between units as per needs Vehicle required for assignment Description: This position requires the candidate to float between units based on needs. Weekends are required every other weekend. There are no on-call requirements. Candidate must have experience with care coordination and managing patient caseloads. They will be expected to manage a patient ratio of 20:1. Behavioral health experience does not count towards the required acute care experience. Candidates must have knowledge of CMS guidelines and be able to determine medical necessity per evidence-based guidelines. Responsibilities include discharge planning and ensuring adherence to HIPAA guidelines. The ability to assess needs and coordinate care for patients is essential. Candidates will work closely within an interdisciplinary team to support patient care. The hospital is a short-term acute care facility with 418 beds. Candidates should have recent experience with the Epic charting system. Commitment includes working 3 out of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, New Year’s Day). Travelers may be expected to float between sister facilities nearby the contracted site. Refusal to float may result in assignment termination.
