Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 Weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 Year in acute care hospital setting License: Active Registered Nurse License Certifications: Basic Life Support (BLS), Case Management Certification Must-Have: - Recent acute care experience in hospital care management/discharge planning - Ability to float to various units based on skill set - Must have a vehicle for assignment Description: The Case Manager is responsible for coordinating patient care and managing discharge planning. The position may require floating between units based on patient needs. Weekend coverage is required every other weekend. On-call duty is not required. The expected patient ratio is 1:20. Candidates should possess skills in care coordination, discharge planning, and medical necessity determination. Familiarity with charting in an electronic health record system is necessary. Behavioral health experience will not be counted as acute care experience. The role necessitates compliance with regulations and guidelines in the healthcare setting. Travelers may be asked to float to different facilities as needed. The position requires a strong focus on service and effective communication with interdisciplinary teams.
