Pay: $2,534.00 weekly

Shifts: M-F 8am-5pm (No oncall)

Key Responsibilities of the RN - Case Management
  • Perform comprehensive assessments of high-risk members.

Requirements of the RN - Case Management
  • RN License for WI
  • Hospice and Wound Care Experience
  • Proficiency in Home Care Home Base (HCHB) or other EMRs like Matrix Care
  • Valid CPR/BLS Certification
  • Valid Driver's License and Reliable Vehicle
  • Valid Car Insurance
  • Skills competency checklist including Elder Abuse, HIPAA, and OSHA training
  • Associates or Bachelor's Degree in Nursing
  • Current, unrestricted RN license
  • Case Management certification
