Shifts: M-F 8am-5pm (Weekend on call rotation)

Seven Healthcare are seeking a dedicated RN - Case Management professional with a specialty in Home Health to join their team on a contract-to-hire basis. This role involves field-based work across several counties, providing personalized care management in a home healthcare setting.

Key Responsibilities of the RN - Case Management
  • Perform comprehensive assessment of high-risk members
  • Collaborate with primary care providers to implement individualized care plans
  • Address and reassess cost-efficient, appropriate levels of care
  • Coordinate care transitions as needed
  • Engage specialty resources for optimal resolution
  • Document clinical, functional, and fiscal outcomes

Requirements of the RN - Case Management
  • RN Licenses in IN or compact license
  • At least two years recent experience in Home Health
  • Proficiency in Wound Care, IV, Drain Care, G tubes
  • HCHB preferred
  • Current, unrestricted state Registered Nurse license
  • Associates or Bachelor's Degree in Nursing or related field
