Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 2 years required License: Active Registered Nurse license Certifications: BLS Must-Have: - Previous charge experience preferred - Experience with Epic charting system required - Travel experience required Description: This is a case management position for a registered nurse. The ideal candidate will have a minimum of two years of experience. First-time travelers are accepted with clinical approval. There are weekend requirements, with two weekends every six weeks. Call is not required for this position. Travelers may need to float between similar units based on need. Local candidates are welcomed to apply. The candidate should submit any requests for time off at the time of application. They will be expected to work three of the five major holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day. Two references covering one year of experience within the last three years are required. The candidate should be familiar with interdisciplinary support services. They must be competent with arranging durable medical equipment and skilled nursing facility transport. Computerized documentation is required in this role. Experience with acute hospital, ambulatory care, and behavioral health is preferred. Familiarity with quality assurance and compliance standards such as CMS and The Joint Commission is beneficial. The candidate should also have knowledge of coding systems, care coordination, and discharge planning.
