Job Title: Case Manager Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: 2 years License: Valid nursing license Certifications: BLS Must-Have: - Previous charge experience - Experience with patient ratios of 20 - Proficient in Epic charting system - Competent in care coordination, discharge planning, and disease management - Ability to perform case management independently Description: This position requires a registered nurse specializing in case management. Candidates must have a minimum of 2 years of relevant experience. First-timers may be accepted with clinical approval. Weekend coverage is required, with 2 weekends every 6 weeks. Floating between similar units may be necessary based on needs. Local candidates are welcome to apply. The role involves working with adults and geriatric patients. Interdisciplinary support will be available. Skills in managing home health, hospice, and long-term acute care are advantageous. Knowledge of disability case management and prior authorizations is essential. Providers are expected to float between facilities within the state as needed. Refusal to float may result in termination of the assignment. Candidates must commit to working 3 of the 5 specified holidays. References covering 1 year during the compliance process will be required. Completion of all required modules is expected within the specified timeframe. Travel experience is required for this position. Proficiency in HIPAA guidelines and CMS regulations is beneficial.
