Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 Weeks Shift: Day Hours per Shift: 8 Experience: 1 Year License: RN Licensure Certifications: BLS Must-Have: Acute Care Experience Discharge Planning/Care Coordination Experience Weekend Availability (every other weekend) Description: This position requires an RN or MSW. The contract length is for 13 weeks. Candidates with 1 year of experience will be considered. First-time travelers are accepted. Acute care experience is a must, along with discharge planning and care coordination experience. Candidates with diverse case management experience, including Med Surg, will be preferred. Mandatory certifications include BLS and RN licensure. The role requires floating between various areas of the facility. Candidates will work in all areas of the hospital. Behavioral health experience does not count as acute care hospital experience. Patient age groups include neonates, adolescents, infants, adults, pediatrics, and geriatrics. Working holidays will be required, with a commitment to work three of the five holiday shifts. References are required that cover the last three years, including charge nurses or leads. All travelers are expected to float between various units within their skill set. Travelers will be required to complete modules before starting and annually. Effective communication devices will be provided for performing duties as assigned.
