Certifications: - Must possess relevant certifications in case management nursing. Must-Have: - Must have experience in assessing patients, care coordination, and discharge planning. - Ability to organize and participate in family meetings is essential. - Experience with health maintenance organizations and independent practice associations is preferred. - Knowledge of drug and alcohol abuse treatment options, supportive services for patients experiencing homelessness, and advance directives is required. - Competence in arranging durable medical equipment, skilled nursing facilities, and ambulance transport independently is necessary. Description: We are seeking a seasoned traveling case manager nurse. The nurse will be responsible for managing a caseload of up to 25 patients daily. The role includes participating in the development of the treatment plan and conducting needs assessments. Nurses will be expected to float between facilities within the state as needed. Weekend work is required every other weekend to meet department needs. The position will not allow for floating to different units outside of case management. Nurses must adhere to all patient safety goals and core measures. Computerized documentation is a necessary part of the job. The environment is supportive with interdisciplinary collaboration, including physical therapy and social services.
