Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: - Minimum 2 years of Acute Care Case Management or Discharge Planning experience required. - Hospital experience is mandatory; behavioral health experience does not apply. - Traveler must be seasoned and have experience with acute care settings. License: - Current state nursing license required. - Pending licenses accepted, but must obtain full licensure before starting. Certifications: - Relevant nursing and case management certifications preferred. Must-Have: - Experience with assessing patients, care coordination, and discharge planning. - Knowledge of community resources, patient advocacy, and utilization review. - Competence in arranging durable medical equipment and transport services. Description: This position involves managing a caseload of patients and coordinating their care. The Case Manager will assess patient needs and develop comprehensive care plans.
