Job Title: RN - Case Management

Location: Charlottesville, VA

Pay: $2,492.80 per week

Shifts: Monday-Friday, 8:00 am-5:00 pm. On-call 1 week monthly.

This role involves coordinating comprehensive care plans in a home health setting. The ideal candidate will cover areas from Culpeper to Charlottesville, providing high-quality care and support.

Key Responsibilities of the RN - Case Management
  • Perform comprehensive assessment of high-risk members.
  • Collaborate with primary care providers to implement individualized care plans.
  • Address and reassess cost-efficient, appropriate levels of care.
  • Coordinate care transitions as needed.
  • Engage specialty resources for optimal resolution.
  • Keep detailed records of clinical, functional, and fiscal outcomes.
  • Identify opportunities for health promotion and illness prevention.
  • Act as a patient advocate, protecting privacy and confidentiality.

Requirements of the RN - Case Management
  • Multi-State Registered Nurse license.
  • Two years of recent experience in home health using HCHB and OASIS.
  • OASIS and HCHB experience required.
  • Case Management certification.
  • CPR/BLS certification.
  • Homecare Homebase experience.
  • TB test.
