The CM department also has 2 clinical coordinators who can assist with tasks that don't require an RN or MSW/LCSW license, such as sending referrals via EPIC, following up with facilities to obtain a bed assignment, delivering DME to the bedside, faxing clinicals to Livanta. Unit based with caseload of 20-23 patients. Responsible for initial assessment within 24 hours, coordination of discharge plan and coordination of all post acute care needs. Will work with a variety of patients of varying ages and with various diagnoses. Will work closely with medical team, insurance case managers as well as Providence UM case managers to safely and quickly discharge patient to the lowest level of care possible.
