Job Title: RN Transfer Center Coordinator Profession: RN Specialty: Transfer Center Coordinator Duration: 13 weeks Shift: 07:00 PM - 07:30 AM Hours per Shift: 12.5 Experience: Minimum 4 years License: Registered Nurse (RN) Description: Facilitates the efficient transfer of patients between facilities. The Transfer Center Coordinator is responsible for coordinating the safe, timely, and efficient transfer of patients into, out of, and within the healthcare system. This role serves as a central point of communication between referring facilities, physicians, nursing leadership, bed management, and ancillary departments. It ensures appropriate placement of patients based on clinical needs, bed availability, and organizational capabilities. The role ensures compliance with regulations and maintains detailed records for quality assurance. Coordinates communication between transferring and receiving medical staff. Arranges required transportation and accommodations for patient transfer. Ensures compliance with state and federal transfer regulations. Maintains detailed records of all transfer activities for quality assurance. Weekend and holiday rotation required.
