Job Overview

We are seeking an experienced Registered Nurse – Case Manager to join a hospital-based Case Management team. The RN Case Manager will be responsible for coordinating patient care, conducting utilization review, and managing discharge planning while ensuring appropriate use of healthcare resources.

The ideal candidate will have strong experience in acute care case management, care coordination, and discharge planning.

Key Responsibilities
  • Conduct patient assessments and care coordination across the continuum of care

  • Manage discharge planning and coordinate post-acute care services

  • Determine medical necessity using evidence-based guidelines (InterQual)

  • Arrange durable medical equipment (DME), SNF placements, and ambulance transport

  • Review benefits eligibility, prior authorizations, and admission criteria

  • Collaborate with interdisciplinary teams including physicians, social services, and therapy departments

  • Participate in family meetings and treatment planning

  • Maintain documentation using Epic EMR

Patient & Unit Details
  • Patient Population: Neonates, Infants, Pediatrics, Adolescents, Adults, and Geriatrics

  • Caseload: Up to 25 patients

  • Patient Ratio: Approximately 1:20
