Location: Winston Salem, NC

Pay: $2,442.40 weekly

Shifts: Monday thru Friday (8am-5pm)

Seven Healthcare are seeking a skilled and dedicated RN - Case Management professional to join their team in Winston Salem, NC. This travel role focuses on home health care, supporting patients across Davidson and Forsyth counties. The position involves working closely with primary care providers to develop and implement comprehensive care plans in a dynamic work environment.

Key Responsibilities of the RN - Case Management
  • Perform comprehensive assessment of high-risk members
  • Collaborate with primary care providers to implement individualized care plans

Requirements of the RN - Case Management
  • Valid RN license for NC
  • Associates or Bachelor's Degree in Nursing or related field
  • Case Management certification
  • Experience with Home Health (HCHB experience), Case Management, Oasis (certification preferred)
