Job Title: RN - Case Management (Travel)

Location: Waterford, WI

Pay: $2442.40 weekly

Shifts: M-F 8am-5pm (No oncall)

Key Responsibilities of the RN - Case Management
  • Perform comprehensive assessment of high-risk members.
  • Collaborate with primary care providers to implement individualized care plans.
  • Continuously reassess appropriate levels of care and address cost-efficiency.
  • Coordinate care transitions and engage necessary specialty resources.
  • Maintain detailed records of clinical, functional, and fiscal outcomes.
  • Identify opportunities for health promotion and illness prevention.
  • Act as a patient advocate, protecting privacy and confidentiality.

Requirements of the RN - Case Management
  • Current, unrestricted RN License for WI.
  • Hospice and wound care experience.
  • Experience with EMR systems; HCHB or Matrix Care preferred.
  • Certified CPR/BLS.
  • Valid driver's license, reliable vehicle, and valid car insurance.
  • Completion of skills competency checklist, elder abuse, HIPAA, infection control, and OSHA training within the past 12 months.
  • Degree in Nursing or related field; Case Management certification preferred.
