Pay: $2,442.40 weekly

Shifts: M-F 8:30 AM - 5:00 PM

Seven Healthcare are seeking an experienced RN for a Case Management position in Savannah, GA. This travel contract role involves working in a dynamic healthcare environment, focusing on providing comprehensive care management to high-risk members.

Key Responsibilities of the RN - Case Management - Travel
  • Perform comprehensive assessment of high-risk members
  • Collaborate with primary care providers to ensure implementation of personalized care plans
  • Address and reassess cost-efficient, appropriate levels of care
  • Implement non-medical support to ensure treatment compliance
  • Coordinate care transitions with a multi-disciplinary team

Requirements of the RN - Case Management - Travel
  • Unencumbered GA RN license
  • Proficient with OASIS documentation and accuracy
  • Experience with HCHB and home health care
  • Associate's or Bachelor's Degree in Nursing or related field
  • Case Management certification preferred
  • Ability to analyze complex medical information and make rational decisions
