Job Title: RN - Case Management - Travel

Location: Pensacola, FL

Pay: $2,442.40 weekly

Shifts: M-F 8am-5pm, Some weekend availability (one weekend per month, one on-call night per week)

This role involves working across a variety of counties including Escambia, Santa Rosa, Okaloosa, and Walton, providing comprehensive care management to high-risk patients. Key Responsibilities of the RN - Case Management
  • Perform comprehensive assessment of high-risk members
  • Collaborate with primary care providers to ensure the implementation of individualized care plans
  • Continuously reassess and address cost-efficient, appropriate levels of care
  • Put non-medical support in place to ensure compliance with treatment plans
  • Coordinate with medical professionals in transitioning patient to appropriate care levels
  • Engage specialty resources to achieve optimal resolutions
  • Maintain detailed records of clinical and fiscal outcomes
  • Identify opportunities for health promotion and illness prevention
  • Act as patient advocate by protecting privacy and confidentiality issues

Requirements of the RN - Case Management
  • Associates or Bachelor Degree in Nursing or related field
  • Current, unrestricted state Registered Nurse license
  • Ability to analyze complex medical information and make rational decisions
  • Competent in Home Care Home Base/Pointcare and Oasis collection
  • Florida License required, will accept compact license
