Job Title: RN - Case Management

Pay: $2,442.40 weekly

Shifts: Monday through Friday, 8 AM - 5 PM, with some call

Key Responsibilities of the RN - Case Management
  • Perform comprehensive assessment of high-risk members
  • Collaborate with primary care providers to implement a tailored and comprehensive care plan
  • Continuously reassess and address cost-efficient, appropriate levels of care
  • Facilitate non-medical support to ensure compliance with treatment plans
  • Coordinate care transitions and engage specialty resources as required
  • Maintain detailed records of clinical, functional, and fiscal outcomes
  • Identify opportunities for health promotion and illness prevention
  • Act as a patient advocate by protecting privacy and confidentiality

Requirements of the RN - Case Management
  • Kentucky RN license
  • Associate or Bachelor's Degree in Nursing or related field
  • Experience in Home Health, OASIS, and HCHB
  • Ability to analyze complex medical information and make decisions
