This role involves performing comprehensive assessments of high-risk members and collaborating with primary care providers to implement individualized care plans. The ideal candidate will have experience in home health and be familiar with HCHB and Oasis.

Key Responsibilities of the RN - Case Management - Travel
  • Perform comprehensive assessment of high-risk members
  • Collaborate with primary care providers to ensure the implementation of individualized care plans
  • Address and continuously reassess cost-efficient, appropriate levels of care
  • Coordinate care transitions and engage specialty resources as needed
  • Maintain detailed records of clinical, functional, and fiscal outcomes
  • Identify opportunities for health promotion and illness prevention
  • Act as a patient advocate by protecting privacy and confidentiality issues

Requirements of the RN - Case Management - Travel
  • Current, unrestricted state Registered Nurse license (NC or Compact License accepted)
  • Associates or Bachelor's Degree in Nursing or related field
  • Case Management certification
  • Ability to analyze complex medical information and make rational decisions
  • Experience in home health, HCHB, and Oasis
