Job Title: RN - Case Management

Location: Camp Hill, PA

Pay: $2,442.40 weekly

Shifts: Monday through Friday 8:00am to 4:30pm; no weekend or on-call

Seven Healthcare are seeking a dedicated RN specializing in Case Management for a travel contract to hire assignment in Camp Hill, PA. This role requires expertise in hospices and using Home Care Home Base (HCHB). The position is hybrid and involves managing patient care across Primary Cumberland/Perry County, with potential travel across other areas as needed.

Key Responsibilities of the RN - Case Management

Requirements of the RN - Case Management
  • RN License
  • Valid Driver's License
  • Valid Car Insurance

  • Associates or Bachelor's Degree in Nursing or related field
  • Ability to analyze complex medical information
  • Case Management certification
