Job Title: Home Health RN Case Manager

Location: Baltimore, MD

Pay: $2,442.40 weekly

Shifts: M-F 8am-5pm

Seven Healthcare are seeking an experienced Home Health RN Case Manager for a contract role in the Baltimore area. This position involves traveling within Baltimore City, Baltimore County, and other potential locations to provide comprehensive care management. Candidates with wound care experience and proficiency in Point of Care/HomeCare HomeBase and Oasis are encouraged to apply.

Key Responsibilities of the Home Health RN Case Manager
  • Perform comprehensive assessment of high-risk members.
  • Collaborate with primary care providers for individualized care plans.
  • Identify and reassess cost-efficient care levels.
  • Coordinate non-medical support for compliance with treatment plans.
  • Facilitate care transitions as needed.
  • Coordinate with healthcare professionals for appropriate care levels.

Requirements of the Home Health RN Case Manager
  • Associate's or Bachelor's Degree in Nursing or related field.
  • Current, unrestricted state Registered Nurse license.
  • Case Management certification preferred.
  • Valid CPR/BLS certification, driver's license, and car insurance.
  • Home health experience and wound care expertise.
  • Familiarity with Oasis and Point of Care/HomeCare HomeBase.
