Job Title: RN Case Management/Discharge Planning Profession: RN (Registered Nurse) Specialty: RN Case Management/Discharge Planning Duration: 13 weeks Shift: 8H Days Hours per Shift: 8 hours Experience: Minimum of 3 years of acute inpatient case management experience, including at least 1 year in a large hospital (500+ beds, Level 1/Academic Medical Center) License: Nursing degree preferred BSN; ADN will be considered Certifications: BLS Certification, CCM preferred but not required Must-Have: - Minimum 3 years of acute inpatient case management experience - Strong computer skills, Epic EMR experience - Acute discharge planning experience - Recent (within the last two years) acute inpatient case management experience Description: Candidates must be highly experienced in acute inpatient case management and discharge planning. Training on the unit will be minimal; candidates will need to pick up quickly and work efficiently. The role involves managing a caseload of 15-20 patients per day and consulting with the social work team, which consists of multiple full-time and part-time social workers. The position covers all inpatient units throughout the facility. There are contracts with local facilities for complex placements, with specific criteria for financial assistance applicable only to uninsured or underinsured patients. The work environment consists of a variety of complex patients. Scrub color is flexible; any color scrubs or business professional attire is acceptable. Consultation with respiratory therapy and pharmacy will be done as needed. There is a 2-week department-specific orientation that includes general facility orientation. RN's will work alongside the case management team and will never work alone.
