Description:
We currently have an opportunity for an experienced RN Care Manager for a 13-week assignment.
This is a 40-hour per week position with variable shifts of either 8:30 am to 4:30 pm or four 10-hour shifts from 7:30 am to 5:00 pm.
There is no call requirement, but weekend and holiday rotation is required.
The RN Care Manager will coordinate concurrent utilization management activities hospital-wide.
This role serves as the main link between the patient, health care team, organization, and payors during the patient's hospital stay.
Responsibilities include:
Conducting assessments of patient needs.
Identifying resources required to transition patients to the next setting.
Securing the necessary resources for patients.
Collaborating, educating, and leading an interdisciplinary team of professionals in daily care rounds, care conferences, and care planning.
Demonstrating excellent communication skills in written, verbal, and electronic systems such as email and software applications used within the facility.
Working with community organizations to secure durable medical equipment, health insurance authorizations, and arrange transportation.
Participating in the length of stay management for inpatient populations.
Possessing knowledge of hospital reimbursement methods and quality measures applicable to the care management department.
Education and experience requirements include:
An Associate or Bachelor degree in nursing.
3-5 years of clinical experience in nursing, home care, hospital case management, or utilization review.
Certification by a nationally recognized case management or utilization review organization is preferred.
