Job Overview

We are seeking an experienced Registered Nurse – Case Manager to join an acute care hospital’s Case Management team. The RN Case Manager will be responsible for coordinating patient care, performing admission reviews, and managing discharge planning while ensuring efficient utilization of healthcare resources.

The ideal candidate will have strong experience in care coordination, utilization review, and discharge planning within an acute care hospital setting.

Key Responsibilities
  • Perform patient assessments and care coordination throughout hospitalization

  • Conduct admission reviews using InterQual criteria

  • Develop and manage individualized plans of care

  • Coordinate discharge planning and post-acute care services

  • Arrange durable medical equipment (DME) and required patient services

  • Collaborate with physicians, nurses, and social services to optimize patient outcomes

  • Review benefits eligibility, prior authorizations, and admission criteria

  • Maintain accurate documentation using Epic EMR
