Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 3 years of inpatient hospital or acute care experience preferred. License: RN state license required. Certifications: BLS preferred. Must-Have: Epic charting experience required. Description: Case Manager RN will oversee guardianships, complex medical cases, and placements. This position involves working with social situations to ensure safe discharges. The role includes providing and coordinating medical home care, placement, and addressing social issues. Responsibilities include conducting case management assessments and care coordination. This position supports a team with interdisciplinary collaboration for comprehensive discharge planning. Candidates must be comfortable floating within the facility as needed. Weekends are not required, but shifts may be available if interested. The average daily census is 15 cases. Travelers will be required to float between facilities as necessary. Clinical services provided will focus on social determinants of health screening and referrals to outpatient resources.
