Job Overview

We are seeking an experienced Registered Nurse – Case Manager to join the Case Management team. The RN Case Manager will be responsible for coordinating patient care, discharge planning, and ensuring appropriate utilization of hospital services while collaborating with interdisciplinary teams to optimize patient outcomes.

This role involves working across multiple areas of the hospital and coordinating transitions of care to the next appropriate level.

Key Responsibilities
  • Coordinate patient care and discharge planning within an acute care hospital setting

  • Conduct patient needs assessments and develop care plans

  • Collaborate with physicians, nurses, and social services to ensure effective care coordination

  • Facilitate discharge planning including arranging home services and DME (Durable Medical Equipment)

  • Monitor patient progress and update plans of care accordingly

  • Ensure compliance with CMS (Centers for Medicare & Medicaid Services) regulations

  • Utilize Epic charting system for documentation

  • Work across hospital departments including both Colby and Pacific campuses

Required Qualifications
  • Active RN License

  • BLS Certification

  • Minimum 1 year of Case Management or Acute Care RN experience

  • Acute Care Hospital Experience

  • Experience with Discharge Planning and Care Coordination

  • Familiarity with Epic EMR
