40 guaranteed hours per week
Weekend rotation: 1 weekend per month
No on-call
Floating required (travelers float first; may float to sister campuses)
Perform acute care case management for hospitalized patients
Complete admission and continued stay reviews
Coordinate discharge planning (home health, SNF, rehab, DME, etc.)
Collaborate with physicians, nursing, social work, and interdisciplinary teams
Ensure compliance with medical necessity and payer guidelines
Facilitate smooth transitions of care to reduce readmissions
Document accurately and timely in the EMR
Function as core staff and integrate into unit culture
