Job Title: RN Case Management Profession: Registered Nurse Specialty: Case Management Duration: 12 months Shift: Monday - Friday Hours per Shift: 08:00 - 16:30 Experience: Minimum 5 years recent RN Case Management experience, with experience in a long-term acute care or short-term acute care setting. License: Must have an active RN license. Certifications: Must have Basic Life Support (BLS) certification through the American Heart Association. Must-Have: Must have recent experience in discharge planning across acute care settings. Strong interdisciplinary team collaboration experience. Description: Coordinates management of care and ensures optimum utilization of resources. Ensures compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment. Responsible for care planning and plan implementation. Evaluates individual patient needs. Enhances the quality of patient management and satisfaction. Promotes continuity of care and cost-effectiveness. Integrates functions of case management and utilization review. Serves as a patient advocate through resource utilization. Addresses the holistic needs of the patient.
