Job Title: RN Case Management Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum 5 years recent RN Case Management experience, with experience in long-term acute care or short-term acute care settings, discharge planning across acute care settings, and strong interdisciplinary team collaboration. License: Must have a Texas RN or multi-state RN license. Certifications: BLS certification through the American Heart Association is required. Must-Have: - Comprehensive assessment and care planning skills. - Experience in care coordination and evaluation of individual patient needs. - Patient advocacy skills in resource utilization and discharge planning. Description: Coordinates management of care and ensures optimum utilization of resources, service delivery, and compliance with external review agencies.
