Travel Nurse RN - Case Manager - $2,274 per week in New York City, NY

Must-Have: - Healthcare Management - RN License - Managed Care Description: The primary goal of the Care Manager is to optimize members' health care and enhance the delivery of care experience. In collaboration with the member's care team, a plan of care with individualized goals and interventions will be developed, implemented, and outcomes evaluated.

Job Title: Care Manager Profession: Care Manager Specialty: ED Duration: Full Time Shift: Monday, Tuesday, Wednesday, Thursday, Friday Hours per Shift: 9 AM - 5 PM Experience: Minimum of five years of clinical experience as a Registered Nurse. Minimum of two years of prior experience in a health care setting, Care Management, or Managed Care setting required. License: RN Licensed as a Registered Nurse in the State of New York required. Certifications: Baccalaureate degree required. PRI and Screen certification within three months of employment is required. Master’s Degree and Bilingual Spanish Certification in Case Management preferred. In collaboration with the member's care team, a plan of care with individualized goals and interventions will be developed, implemented, and outcomes evaluated. Computer literacy is required. Ability to proficiently read and interpret medical records, claims data, pharmacy, lab reports, and prescriptions is necessary. Leadership experience is preferred.
