Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 6 months Shift: Day shift Hours per Shift: 8 hours Experience: - Minimum 2 years of Acute Care Case Management or Discharge Planning experience required. - Hospital experience is required; behavioral health experience does not apply. License: - California state license required. - Pending licenses accepted, but must be licensed upon start date. Certifications: - Applicable certifications based on specialty and experience. Must-Have: - Experience in care coordination, discharge planning, and utilization review. - Ability to organize and participate in family meetings. - Competence in arranging durable medical equipment (DME), skilled nursing facility (SNF), and ambulance transport independently. Description: The position involves managing case responsibilities within an acute care facility. Daily patient assignments may involve a caseload of up to 25 patients. Traveling professionals are expected to float between facilities as necessary. Participation in the development of treatment plans is a fundamental duty.
