Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 Weeks Shift: Day Hours per Shift: 8 Experience: Minimum 2 years of experience required License: Must have a DC RN license in hand prior to start date Certifications: BLS Must-Have: - Experience as a complex Case Manager - Experience in Care Coordination - Experience in Discharge Planning - Knowledge of CPT codes, DRG, HEDIS measures, and NCQA Description: Must have experience as a complex Case Manager, preferably with NCQA experience. Position may require field visits two days per week. Mileage will be reimbursed for field visits. Candidates with a pending DC RN license will be accepted, but must have the license prior to the start date. BLS is required. Two years of experience in acute hospital care, long-term acute care, rehabilitation, or skilled nursing is necessary. Experience with admission criteria and care coordination is required. Candidates should be familiar with discharge planning and utilizing InterQual and Milliman guidelines. Previous charge experience is preferred. Knowledge of regulatory requirements, including OSHA procedures and Joint Commission standards, is required. Candidates must be able to care for a variety of patient demographics, including pregnant persons and geriatric patients. The role involves making calls to enrollees to complete assessments and develop individualized care plans. Collaboration with multiple stakeholders is essential for the successful execution of care plans. Documentation must comply with contract requirements and reflect care coordination standards. Proficiency with computer systems and Microsoft Suite is necessary. Dress code is business casual.
