Job Title: Case Manager Profession: Registered Nurse Specialty: Care Management Duration: 12 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 3 years License: GA or Compact license required Certifications: BLS Must-Have: - Ability to assess, evaluate, and drive the discharge plan. - Experience in coordinating services to the patient's appropriate level of care. - Knowledge to ensure that the treatment plan is initiated, evaluated, and re-evaluated daily. - Capability to monitor length of stay and identify barriers to discharge. - Familiarity with regulatory guidelines regarding patient choice and related notices. Description: This position requires the ability to assess, evaluate, and drive the discharge plan for patients. You will coordinate services to ensure patients receive appropriate levels of care. The treatment plan must be initiated, evaluated, and re-evaluated daily. You will need to monitor the length of stay and identify any barriers to discharge. Adherence to regulatory guidelines regarding patient choice and notifications is essential. The schedule is Monday through Friday, with no weekend shifts required. Floating within the facility/scope of practice may be necessary as needed. Clinical screening will determine the clinical fit for candidates. Compliance with all onboarding requirements is necessary to start the contract on time. If more than one major holiday falls during the assignment period, you will be expected to work them. Candidates must have a minimum of three years of experience, as first-time travelers are not accepted.
