Key Responsibilities of the RN - Case Management
  • Perform comprehensive assessments of high-risk members
  • Collaborate with primary care providers for individualized care plans
  • Identify and address cost-efficient, appropriate levels of care
  • Coordinate with community resources for treatment plan compliance
  • Manage care transitions with team coordination
  • Engage specialty resources as needed
  • Maintain detailed records of patient outcomes
  • Promote health and identify opportunities for illness prevention
  • Act as a patient advocate, safeguarding privacy and confidentiality

Requirements of the RN - Case Management
  • Kentucky license
  • Associates or Bachelor's Degree in Nursing or related field
  • Current, unrestricted state Registered Nurse license
  • Home Health Experience, OASIS, HCHB
  • Ability to analyze complex medical information and make rational decisions
