Job Title: RN Hospice Case Manager Profession: RN Specialty: Hospice Care Duration: 13 weeks Shift: 8 Hour Days Hours per Shift: 8 Experience: Hospice experience required License: Registered Nurse License Certifications: HCHB experience required Must-Have: Hospice experience Description: Identify the patient's and family's physical, psychosocial, environmental, safety, and developmental needs. Individualize the patient's plan of care based on the needs identified. Document problems, nursing assessments, appropriate goals, care provided, care interventions, and patient and family outcomes achieved from intervention and care provided. Assess all aspects of the patient's pain and develop an individualized pain management plan. Anticipate, prevent, and treat undesirable symptoms or secondary symptoms. Participate in the interdisciplinary team meetings to establish, review, and revise the plan of care for each patient, including exchange of information, review of problems, and sharing of professional support. Maintain up-to-date patient records to ensure that problems, plans, interventions, and goals are accurately and clearly stated, and changes are reflected as they occur. Teach appropriate interventions, medications, effects, and side effects to the patient, caregiver, volunteers, and others as appropriate. Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure, and effective grieving for patients and families. Be responsible for the maintenance of electronic medical records, assuring accuracy, completeness, and compliance with regulations, certification standards, legal, and ethical standards. Identify and address comfort care needs. Support, instruct, and educate the patient, family, and caregiver. Coordinate all patient and family services. Maintain the dignity of the dying patient. Support the patients' and families' unique spiritual and cultural beliefs. Provide holistic, family-centered care across treatment settings to improve the quality of life. Consult and collaborate with the interdisciplinary team and others involved in the patient's care. Provide care consistent with national standards of practice for hospice programs.
